Office Policies
We are pleased to welcome you as a patient. Our primary mission is to deliver the best and most comprehensive oral and maxillofacial surgery care available. The purpose of this policy is to empower you by giving you a clear understanding of your financial responsibility with regard to any and all shared costs, co-pay amounts, deductibles, and any balance not paid by your insurance or employer plan.
As a courtesy to you, we will happily submit your insurance claim. It is your responsibility to know what your insurance benefits are. We do our best to estimate your insurance coverage but please remember that it is just an estimate and ultimately, you are responsible for any unpaid balances. Payment of your insurance co-payment is due and collected on the date of service.
We are happy to offer these choices so that you can select a payment option that best fits your needs.
Payment Options
Self Pay: This includes money orders and personal checks and is due at time of service.
Insurance: We will bill your insurance as a courtesy to you, but you are responsible for any insurance co-pay on the day of service.
Easy-Pay Plan: This program allows us to capture your credit card on file. We will give you a courtesy call once your insurance has paid. You can authorize the charge or pay by check at that time.

Your dental benefits are based on the agreement your employer negotiated with your insurance carrier. Please review the insurance benefit booklet provided by your employer to better understand the benefits they make available as part of your insurance coverage. The patient payment portion covered for procedures varies depending on the coverage provided by your employer. An estimate of the amount covered by your insurance company will be provided at the time of your treatment, based on the information they provide to us. The estimate is never a guarantee of benefits or the amount that will be paid. We will file all insurance claims as a courtesy to our patients. This does not however, transfer the responsibility of your financial obligation to the insurance company. If the amount paid by the insurance company is less than or greater than the estimate, then you will be billed the difference or issued a credit on the account in the event of an overpayment.
